DRUG TESTING CONSENT AUTHORIZATION


It is the policy of the Company that all applicants for employment will be required to submit to drug testing. Positive results for any of the drugs screened will result in that applicant's automatic disqualification for employment.


I hereby voluntarily authorize International Tank Service, Inc. as part of the company's per-employment inquiry, to conduct a urine drug test and to use the results of that test for decisions relating to my employment, in accordance with the Company's Drug and Alcohol Policies.

I understand that if I am hired, I am free to resign at any time, with or without cause and without prior notice, and the Company reserves the same right to terminate my employment at any time, with or without cause and without prior notice, except as may be required by law.
